COBRA

COBRA benefits gives a dependent the opportunity to continue dental and optical benefits for a cost, after a qualifying event (Member Passing, Age Out, Not in School, Divorce etc.)

(Children are covered for prescriptions until 26 years of age as long as the Member remains Active)

COBRA must be picked up within 60 days of the qualifying event effective the first of the following month of the event.  These benefits can be maintained for up to 36 months. 

Initial payment must be made by check with application, additional payments can be made by check or online by credit card through this website.
