What Benefits are handled through the DEA?
The DEA handles your Prescription Drugs, Dental and Optical. Your Medical Insurance is handled through City Health Benefits Active 646-610-5122 or Retired 212-513-0470.
Divorce/Removal of Domestic Partner
Upon a divorce/ Termination of Domestic Partnership your ex-spouse/partner must be removed from the benefits immediately as they are no longer entitled to benefits. If they are not removed and there is usage after the date of divorce/termination you will be responsible for any payment made on their behalf.
If you are removing a Domestic Partner you must submit a Termination of Domestic Partnership Form. This can be obtained from your county clerks office.
An ex-spouse/partner is entitled to COBRA if interested. This allows them to continue dental, optical and prescription benefits for a cost.
Who covers vaccinations?
Vaccinations are covered under the medical plans as preventative for all non-Medicare eligible members.
Upon going on to Medicare vaccinations are either covered under Medicare part B or out of your pocket.
Do you cover Baby Formula?
No baby formula is considered a food supplement, and our Prescription Plan only covers medications. You can try through your medical carrier for coverage, or it would be out of your pocket.
